You can use this html and php file.
you can also visit this site for your answer http://www.javascript-coder.com/html-form/html-form-email.phtml
Html File
<html>
<head>
<title>my first html document</title>
</head>
<body>
<div id="wrap">
<form name="contactform" method="post" action="sending_email.php"> <!-- you must use this php file to send mail -->
<label for="first_name">First Name *</label>
<input type="text" name="first_name"/><br/>
<label for="last_name">Last Name *</label>
<input type="text" name="last_name"/><br/>
<label for="email">Email Address *</label>
<input type="text" name="email"/><br/>
<label for="telephone">Telephone Number</label>
<input type="text" name="telephone"/><br/>
<label for="comments">Comments *</label>
<textarea name="comments"></textarea><br/><br/>
<input type="submit" value="Submit">
</form>
</div><!--end wrap -->

</html>

php file
<?php
if(isset($_POST['email'])) {

// EDIT THE 2 LINES BELOW AS REQUIRED

echo "These errors appear below.<br /><br />";
echo $error."<br /><br />";
echo "Please go back and fix these errors.<br /><br />";
die(); // the script stops here if error occurs
}

// validation expected data exists
if(!isset($_POST['first_name']) ||
