Form to email with attachment

I have been working on a website for my wife's company but I am not sure how to get my page to email a form with an attachment. I want people to be able to fill out a little information and email their resume. Can someone please tell me what I need to do to get it to wok.

You should use enctype="multipart/form-data" attribute for <form> tag if you wan to upload something using a <form>.
In fact the code doesn't seems good.
